Cancel your license to practice

If you are no longer practicing pharmacy in Rwanda or planning to do so in the near future. You can also apply to temporarily cancel you license to practice pharmacy professional in Rwanda.

To apply for cancellation of  your license, please follow the steps below:

1. Complete and submit the form below
2. Write a letter to the Chairperson of NPC stating the reason why you are applying for voluntary cancellation of license to practice
